APPOINTMENTS
Our office is currently accepting new patients. Please call us at 912-877-3070 to schedule an appointment. If you are unable to keep your appointment, please notify us within 24-hours to reschedule.
WHAT TO BRING TO YOUR FIRST APPOINTMENT
Please be sure to bring the following items when you visit our practice for your initial visit:
- Insurance Information/Card
- Photo Identification
- Any medical records pertaining to your visit
- Method of payment (Co-pay)
Please arrive 15 minutes before your scheduled appointment time to complete the necessary paperwork. If you would like to complete your paperwork prior to your appointment, then print and fill the appropriate forms below and bring them to your appointment.
INSURANCE
Upon your visit to our practice, please provide us with your current insurance card/information. We accept most dental plans, and as a courtesy, will file dental insurance claims for you.
PAYMENT
Payment is due on the day of service. This includes any co-payments or deductibles (amounts not covered by your insurance).
For your convenience, our office accepts cash, Visa, Discover, MasterCard, & CareCredit.
CareCredit is a credit card program offered by a third party. Finance your treatment costs with CareCredit and receive care immediately! Access the CareCredit (www.carecredit.com) website to obtain more information.

Yes, we accept VA dental community care patients. Prior to your visit, you must contact your VA representative to have your dental referral sent to our office.
